Address Verification

To verify eligibility for carpool registration, the following items are required for each person in the carpool*:

An image of a Government-issued photo ID (driver's license)
Proof of current residency (This can be an image of a lease agreement, mortgage statement, or utility bill with current address. We do not need to see the entire statement or utility bill, just an image that clearly shows the current home address)

*A parent and child do count as a carpool. The child does not need to provide a photo ID or proof of residency.

Please attach photos, pdfs, jpgs, etc. of the ID and proof of residency to this in-take form to be verified. Any carpoolers for whom verification isn't provided will be contacted by Move Minneapolis to make arrangements for this verification step.
